Job summary

South Middlesex Opportunity Council, Inc. is seeking a Case Manager for a 19-bed program serving individuals in early recovery in Framingham, MA. The role involves intake, service planning within 2 days, daily client contact, and coordination with a co-case manager to support long-term recovery.

You will engage clients in treatment, arrange referrals, and monitor progress with detailed case notes and monthly reporting. A strong emphasis is placed on advocacy and community outreach.

Qualifications

• Bachelor’s Degree or related experience.
• Experience working with homeless or disadvantaged population.
• Knowledge of 12 step recovery program.
• Excellent written and oral communication.
• Must have experience with Word and Excel.
• Valid driver’s license.

Responsibilities

• Complete assessment with all clients entering the program as assigned.
• Develop Individual Service Plan with each participant within 2 days of entry into the program.
• Maintain daily contact with all clients in the building and establish regular contact.
• Maintain appropriate files and case notes for each participant.
• Coordinate program duties with co-case manager and ensure effective communication.
• Outreach to community providers to market the program and establish client referrals.
• Co-facilitate weekly house meetings and participate in engagement team meetings.
• Ensure accurate statistical data is maintained and reported monthly.
