Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to collaborate with teams across the organization to promote long-term solutions for individuals and families experiencing homelessness. You will develop care plans, coordinate services, monitor progress, and maintain documentation with trauma-informed practices.

Responsibilities include outreach, crisis intervention, referrals to housing and health services, and advocacy to secure benefits.

Qualifications

• Must have experience managing a diverse caseload.
• Strong interpersonal communication and collaboration with partners.
• Familiarity with motivational interviewing and trauma-informed care preferred.

Responsibilities

• Manage a caseload and coordinate services for clients.
• Develop and monitor individualized care plans and referrals.
• Maintain accurate documentation in the EHR and other systems.
• Provide advocacy and crisis intervention as needed.
