Community Mental Health Affiliates, Inc. (CMHA) is seeking a full-time Case Manager for the Behavioral Health Home in New Britain, CT.

You will support clients with mental health and co-occurring needs, developing person-centered recovery goals and facilitating access to community resources. Responsibilities include maintaining caseloads, documenting progress, and collaborating with the treatment team to ensure seamless service delivery.

Qualifications

• Bachelor’s degree in mental health field preferred or equivalent experience.
• Valid Connecticut driver’s license and reliable vehicle.
• Experience in case management, mental health and co-occurring conditions (1+ year).
• Proficient with Microsoft Office (Word, Outlook, Excel, PowerPoint).
• Experience with Electronic Medical Records (EMR).

Responsibilities

• Maintain caseload and develop individualized treatment plans.
• Provide direct care through education, skills training, and support.
• Educate clients on resources, finances, housing, and daily living activities.
• Document progress notes, treatment plans, and monthly case notes.
• Coordinate with treatment teams and external providers; may transport clients.
