Job summary

Mental Health Association seeks a Case Manager to provide trauma-informed, person-centered support to individuals in recovery moving from homelessness into studio apartments within a congregate housing site. The role includes on-site work at Chestnut Crossing, with substantial time spent in the community and occasional travel to the main office in Chicopee.

Qualifications

• Valid driver’s license and reliable transportation.
• Must be 21 or older and able to obtain CPR/First Aid certifications.
• Minimum of six months of relevant experience.
• Experience with mental health, substance use, or housing instability preferred.

Responsibilities

• Coordinate documentation for program entry, including homelessness verification, eligibility screenings, and IDs.
• Complete initial Adult Comprehensive Assessments, updates, and service plans.
• Develop Individualized Action Plans (IAPs) with participant input and strengths.
• Implement strategies in the IAP and monitor progress in records.
• Develop, monitor, and revise safety plans with participants and team.
• Assist participants with transition to stable housing.
• Support tenants with housing responsibilities and communications with management.
• Help participants pursue employment, vocational training, or education.
• Educate and assist access to public benefits and resources.
