Job summary

Hogar, Inc. is seeking a dedicated Case Manager to support residents in a Supported Housing Program in New York.

You will perform initial and ongoing assessments, develop and review individualized service plans, and coordinate supportive services tailored to each resident's goals. Responsibilities include intake, monthly assessments, service-plan development within 28 days, and advocacy to ensure access to benefits.

Qualifications

• Bachelor's degree in a human services field.
• Related experience and work with people with mental illness.
• Ability to conduct assessments and develop individualized service plans.

Responsibilities

• Participate in intake and initial assessment of residents for admission to the program.
• Conduct at least monthly assessments of residents to determine functioning, needs and strengths.
• Develop initial individualized service plans within 28 days after admission with residents and involve families as needed.
• Ensure service plan objectives and services align with resident goals and provide needed skill training.
• Record progress of residents within 25 hours of services delivery and maintain current case records.
• Attend case conferences and act as liaison with other service providers to ensure continuity of care.
• Assess income assistance needs, advise on requirements, assist with applications, and advocate for timely benefits.

Education

Bachelor's Degree in a Human Service Field
