ChenMed seeks a Primary Care Physician to deliver high-quality, compassionate care for adults, managing a patient panel up to 450 and leading a collaborative care team. You will coordinate with hospitalists and other health professionals to optimize outcomes and integrate risk-based, value-driven care.

Qualifications include MD/DO in internal medicine, family medicine, or geriatrics, a current state license, and board eligibility.

Competenze

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
Must obtain State Medical License in the practicing state.
Board certification preferred; Board eligibility required.
DEA number for controlled substances.
BLS certification within first 90 days of employment.

Mansioni

Provide care for older adult patients with acute and chronic illnesses.
Assume full accountability for patient care and outcomes; seek specialist consultation as needed.
Collaborate with hospitalists when patients are hospitalized.
Manage assessment, diagnosis, treatment, education, health promotion and care coordination.
Lead the care team (MA, care facilitator, care coordinator) for patients visiting the office.
Engage with transitional care teams for non-ambulatory patients (hospital, SNF, LTC, homebound).
Take active role in center management, recruiting patients and providers as needed.
Supervise or collaborate with APPs and remain accountable for their actions.
Perform other duties as assigned by management.
