Job summary

ChenMed invites applications for a Primary Care Physician (MD/DO) to join our physician-led, mission-driven team. The PCP will manage a panel of up to 450 patients, lead a care team including medical assistant, care facilitator and care coordinator, and collaborate with hospitalists and other care professionals to ensure high-quality, cost-effective care.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar.
• Active State Medical License in good standing.
• Board certification preferred; Board eligibility required.
• Current DEA number for controlled substances (II-V).
• BLS certification within first 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take full accountability for patient care and outcomes; seek consults as needed.
• Engage with hospitalists when patients are in hospital.
• Manage assessment, diagnosis, treatment, and care coordination.
