Job summary

ChenMed LLC is seeking a Primary Care Physician to deliver high‑quality care to older adults in a value‑based, outcomes‑driven model. You will lead a care team and coordinate with hospitalists, aiming for optimal patient outcomes and cost‑effective care.

The role requires MD/DO in relevant specialties, state licensure, DEA and BLS, with strong leadership and commitment to patient‑centered care. Expect to work within a multidisciplinary team across ChenMed centers.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics.
• Board certification preferred; active medical license required in the applicable state.
• Current DEA number and BLS certification within 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Take accountability for patient care and outcomes; consult specialists as needed.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Collaborate with hospitalists and engage in risk and quality management programs.
