Job summary

ChenMed is seeking a Primary Care Physician to deliver high-quality care for seniors in our outcomes-focused model. You will manage a panel of up to 450 patients, act as a health coach, and lead a care team to achieve quality and cost-efficiency goals.

You will collaborate with hospitalists, specialists, and care teams, ensuring compliance with guidelines and participating in risk and quality management programs. A state license and board certification are preferred.

Qualifications

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar
Board certification/eligibility in preferred specialty
Current State Medical License or eligibility
DEA number for controlled substances

Responsibilities

Independently provide care for older adult patients with acute and chronic illnesses
Take accountability for patient care and outcomes; consult specialists as needed
Engage with hospitalist when patients are in hospital
Lead care team including medical assistant, care facilitator, and care coordinator
Coordinate care for patients in hospital, SNF, LTC, or homebound settings
Support center operations and recruitment efforts as needed
Collaborate with APPs and supervise when applicable
Perform other duties as assigned by management
