POSITION SUMMARY

The Lead, Primary Care Physician (Lead PCP) is a key clinical leadership role demonstrating accountability for patient outcomes, coaching for health, simplifying care processes, and leading team meetings. The role involves managing a panel of at least 360 patients, leading clinical and operational meetings, fostering a positive culture, and collaborating closely with leadership and care teams to meet quality and operational metrics.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

• Provide comprehensive care for patients with acute and chronic illnesses, especially in older adults.
• Take full accountability for patient care and outcomes, consulting specialists as needed.
• Engage with hospitalists and coordinate care across settings.
• Lead care teams and participate in clinical meetings and quality programs.
• Support center management and participate in recruiting efforts.
• Be accessible to patients, providing timely responses to their needs.
• Uphold high standards of evidence-based medicine, considering lifestyle and nutrition factors.
• Develop and improve leadership skills and influence positive health behaviors.

KNOWLEDGE, SKILLS AND ABILITIES:

• Proficiency in Microsoft Office and other relevant applications.
• Willingness to travel locally and nationally as needed.
• Fluency in English; additional languages a plus.
• Ability to exercise independent judgment.

EDUCATION AND EXPERIENCE CRITERIA:

• MD or DO in relevant specialties, with current licensure.
• Minimum of 1-year clinical experience in geriatrics, adult, or family practice preferred.
• Board certification or eligibility; commitment to maintaining certification.
• DEA registration and BLS certification required within specified timelines.
