ChenMed is expanding and seeking a Primary Care Physician to deliver high-quality, outcomes-driven care to seniors in a physician-led team. You will manage a patient panel, lead a care team, and coordinate with hospitalists to ensure continuity of care.

Qualificações

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar.
• Must obtain State Medical License or already have one in the state(s) you work.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar; board eligible.
• Maintain board certification through MOC, CME or retaking exams as required.
• Must have a current DEA number for controlled substances.
• BLS certification from AHA/Red Cross within 90 days.

Responsabilidades

• Provide care for older adult patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes and seek specialist consult when needed.
• Engage hospitalists when patients are in hospital.
• Coordinate care, educate, promote health, and document care.
• Lead care team including medical assistant, care facilitator, and care coordinator.
• Engage with transitional care for homebound or hospitalized patients.
• Recruit patients and providers as needed; cover for other providers.
• Supervise APPs and ensure accountability for their actions.
