ChenMed is seeking a Primary Care Physician (PCP) to join our physician‑led, value‑based care team focused on seniors. You will care for a panel of up to 450 patients, deliver high‑quality outcomes, and lead a care team including medical assistants, care facilitators and coordinators.

As a PCP at ChenMed you will collaborate with hospitalists, participate in risk and quality programs, and drive patient‑centered care while maintaining autonomy and professional growth within a supportive culture

Qualificações

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• State medical license required for the practicing state(s).
• Board certification preferred; board eligibility required.
• Current DEA number for controlled substances.
• BLS certification within 90 days of employment.

Responsabilidades

• Independently provides care for acute and chronic illnesses in older adults.
• Accountable for patient care and outcomes; consults specialists when needed.
• Engages hospitalists for inpatient care and coordinates transitions.
• Leads care team including medical assistant, care facilitator and coordinator.
• Engages in recruiting patients and additional providers as needed.
• Collaborates with APPs and adheres to guidelines and risk programs.
