Job summary

ChenMed LLC is seeking a Primary Care Physician (PCP) to provide comprehensive care for an older adult patient panel, up to 450 patients, and to lead a care team in a physician-led, mission-driven primary care setting. The PCP will be accountable for outcomes, collaborate with hospitalists when needed, and help coordinate care across settings.

MD or DO in Internal Medicine, Family Medicine, or Geriatrics is required, with board certification preferred.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics is required.
• State Medical License is required or able to obtain.
• Board certification preferred; Board eligibility is acceptable.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
