Qualifications

• High School Diploma or equivalent plus 3–5 years of clinical case management or care coordination experience.
• Experience using EHR systems and Microsoft Office (Outlook, Word, Excel).
• Strong communication, organizational, and patient service skills with ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, and treatment readiness.
• Monitor progress, address barriers to care, and collaborate with cross-functional teams.
