Job summary

Greenbrook is seeking a dedicated Case Manager to serve as the primary connection for patients throughout their treatment journey. You will coordinate care, manage scheduling, assist with insurance and authorizations, and help patients overcome barriers to access.

The role requires 3–5 years in clinical case management, proficiency with EHR systems and MS Office, and strong communication skills. Location is clinic-based in Annapolis, MD with growth opportunities in a collaborative team.

Qualifications

• High School Diploma or equivalent required; 3–5 years in clinical case management or related healthcare coordination experience.
• Experience using EHR systems and Microsoft Office applications (Outlook, Word, Excel).
• Strong communication, organizational, and patient service skills with the ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, and specialty pharmacy activities.
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams.
• Maintain accurate documentation and patient records within CRM and EHR systems.
• Respond to patient inquiries with empathy and professionalism; deliver exceptional service.
• Support high-quality outcomes while ensuring compliance with requirements.
