Job summary

Greenbrook Mental Wellness Centers in Columbia, MD, is seeking a Case Manager for TMS & Spravato patient care coordination. You will be the primary contact for patients, guiding their treatment journey with empathy and efficiency.

Coordinate appointments, authorizations, specialty pharmacy activities, and documentation across CRM and EHR systems while collaborating with cross-functional teams to maintain treatment readiness and access to care.

Qualifications

• High School Diploma or equivalent with 3–5 years in clinical case management or care coordination.
• Experience using EHR systems and Microsoft Office (Outlook, Word, Excel).
• Strong communication and organizational skills with the ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and treatment readiness requirements.
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track.
• Maintain accurate documentation, financial estimates, and patient records within CRM and EHR systems.
• Respond to patient inquiries with empathy, professionalism, and urgency while delivering exceptional service.
• Support high-quality patient outcomes while ensuring compliance with operational, coverage, and treatment requirements.
