Job summary

Greenbrook Mental Wellness Centers is seeking a compassionate Case Manager to support patients through TMS and Spravato care coordination in Delray Beach. You will manage care coordination, appointments, insurance authorizations, and documentation to ensure smooth treatment journeys.

The role emphasizes patient advocacy, organized workflows, and collaborative problem-solving in a fast-paced clinical setting.

Qualifications

• High School Diploma or equivalent required with 3–5 years in clinical case management or related healthcare coordination.
• Experience using EHR systems and MS Office applications.
• Strong communication, organizational, and patient service skills with ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, and treatment readiness.
• Monitor patient progress and address barriers to care with cross-functional teams.
• Maintain accurate documentation and patient records in CRM and EHR systems.
• Respond to patient inquiries with empathy and professionalism.
• Support high-quality outcomes while ensuring compliance with requirements.
