The position requires 40 hours per week, full-time telework with 50-75% travel around Chicago, IL. A bachelor’s or master-level degree in Behavioral Health or Human Services is required, along with experience in electronic documentation and MS Office.

Formación

• Bachelor's degree or non-licensed master level clinician in Behavioral Health or Human Services.
• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% around Chicago, IL; reliable transportation required.

Responsabilidades

• Evaluate members’ needs and eligibility and propose care strategies.
• Identify high-risk factors and coordinate appropriate referrals to care management.
• Coordinate and monitor care plan activities and progress.
• Collaborate with case managers, supervisors, and Medical Directors to improve outcomes.
• Engage members to participate in decision-making and lifestyle changes.
