Eligible applicants will have 2+ years in Behavioral Health or Social Services, strong Microsoft Office and electronic documentation experience, and a Bachelor’s in Behavioral Health or related field.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• Must be willing and able to travel 50-75% of the time in and around Southern Chicago, Illinois. Reliable transportation required.
• Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluation of Members: through care management tools and data review, evaluate needs/eligibility and recommend approach to case resolution.
• Identify high risk factors and service needs; refer to clinical case management or crisis intervention as appropriate.
• Coordinate and implement care plan activities and monitor progress.
