Qualifications

• 2+ years of experience in Behavioral Health, Social Services or related field.
• Must be willing to travel 50-75% in specified Illinois counties.
• Experience with electronic documentation and Microsoft Office applications required.

Responsibilities

• Evaluate members’ needs/eligibility and recommend case resolution.
• Identify high-risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Consult with care managers and medical directors to optimize outcomes.
• Escalate quality of care issues through established channels.
• Negotiate options and services to meet members’ benefits and healthcare needs.
• Engage members to support healthy lifestyle changes and informed decision-making.
• Participate in team meetings and mentor colleagues.
• Assist members in working with their providers for care decisions.
• Document care processes in compliance with regulatory guidelines.
