Job summary

CVS Health Corporation is seeking a Case Management Coordinator to support members by coordinating appointments, accessing benefits, and educating on care options. This full-time telework role involves 50-75% travel around Chicago, IL, with standard M-F hours.

Responsibilities include evaluating member needs, coordinating care plans, and ensuring quality of care through collaboration with clinical teams. A Bachelor’s or master-level Behavioral Health/Human Services degree is required.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.

Responsibilities

• Evaluate members and assess needs/eligibility to recommend care plan and available programs.
• Identify high risk factors and escalate to clinical case management or crisis intervention as needed.
• Coordinate and implement assigned care plan activities and monitor progress.
• Consult with case managers, supervisors, Medical Directors to overcome barriers to goals and outcomes.
• Identify and escalate quality of care issues through established channels.
• Negotiate options and services to meet member benefits and healthcare needs.
• Motivate and coach members to engage in healthy lifestyle choices.
• Participate in team meetings and provide peer mentoring and training.
• Assist members in participating with providers in healthcare decisions.
• Document care through case management and quality management processes.
