Job summary

Qualifications

• Active RN license required.
• 3+ years of clinical experience (hospital/acute care/home health).
• Current, unrestricted RN license; compact licensure recognized.

Responsibilities

• Provide telephonic care and direction to patients/members.
• Assist provider/team with care coordination activities.
• Perform telephonic triage to determine appropriate level of care.
• Collaborate with patients, APCs, and community to respond to condition changes.
• Document all clinical information and telephonic assessments in the EMR.
• Coordinate follow-up for members in the ER.
• Support onboarding of new clinical staff under supervision.
