Job summary

Central Ohio Primary Care is seeking a Triage Specialist to perform telephonic nursing triage for acute and chronic concerns, using EPIC for documentation and coordinating with the care team. The role is Full Time with standard daytime hours in Westerville, OH, with 4-10 hour shifts and every-other weekend availability.

Qualifications

• Active RN listening, communication, and documentation skills required.
• Bachelor’s degree in Nursing and Ohio RN license required.
• Minimum 2 years RN experience in hospital or medical practice setting; telephonic triage preferred.

Responsibilities

• Conduct telephone triage for patient health complaints using an electronic triage tool.
• Navigate multiple electronic platforms to assess health concerns, chart, schedule PCP and ancillary services.
• Document nursing triage notes in the EHR for clear documentation.
• Record call details in tracking system for department data collection.
• Identify and close patient care gaps.
• Build positive working relationships with COPC staff, offices and physicians to deliver high-quality patient care.
