Qualifications

• Active RN license required.
• 3+ years of clinical experience (hospital/acute care/home health).
• Current, unrestricted RN license; compact licensure recognized.

Responsibilities

• Collaborate with patients, APCs, and community to respond to condition changes.
• Document all clinical information and telephonic assessments in the EMR.

About This Role

The Telephonic Urgent Care RN provides telephonic care and direction to patients/members, working in collaboration with an interdisciplinary care team. This role is ideal for RNs who thrive in a home-based environment and are committed to delivering high-quality patient care.

Highlights

• Pay: $29 - $52/hour
• Location: Columbus, OH
• Schedule: Full Time, Monday through Friday, 8:00 AM to 5:00 PM, with required rotating weekend coverage

What You'll Do

• Provide telephonic care and direction to patients/members during various hours
• Assist the provider/team with care coordination activities
• Perform telephonic triage of incoming calls to determine the correct level of care
• Collaborate with patients/members, APCs, and community members to respond to changes in condition
• Document all clinical information and telephonic assessments in the appropriate tools
• Assist in the coordination of follow-up for members in the ER
• Participate in onboarding of new clinical staff under the direction of the CSM
• Escalate appropriate calls to the Optum APC
• Document all interactions within the electronic medical record

Requirements

Education:

• Active Compact RN license required

Experience:

• 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management position

Licenses & Certifications:

• Current, unrestricted RN compact license
• Additional licensure in assigned states must be obtained within 6 months of hire
