Job summary

Optum is seeking an Urgent Care Registered Nurse to work remotely across the United States. This role delivers telephonic and virtual care to enrolled patients as part of an integrated care model, coordinating with interdisciplinary teams to support patients in various settings.

You will be responsible for care coordination, triage, and documentation under the Urgent Care Clinical Services Manager, with rotating weekend coverage and a two-week schedule.

Qualifications

• Must hold a current, unrestricted RN compact license.
• Additional licensure in assigned states must be obtained within 6 months of hire.
• 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management position.
• Dedicated space for home office setup.
• Access to high-speed internet services.
• Computer/typing proficiency to enter and retrieve data in electronic clinical records.
• Proficient with Microsoft Word, Outlook, and Excel.
• Solid problem-solving skills.
• Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others.
• Ability to flex with change and perform positively and efficiently in production driven environment.
• Ability to work scheduled shifts in accordance with scheduling policies.

Responsibilities

• Care Coordination: Assist the provider/team with care coordination activities and telephonic assessments.
• Team Support: Coordinate follow up of members in the ER and participate in onboarding of new clinical staff.
• Documentation: Document all clinical information in the EMR.
• RN to APC Hand Off: Escalate appropriate calls and document interactions in the EMR.
