Job summary

Central Ohio Primary Care Physicians,Inc is seeking a Triage Specialist-RN (Day Shift) for a hybrid schedule in Westerville, OH. You will perform telephonic nursing triage, document interactions in EPIC, schedule PCP appointments, and collaborate with the care team to close patient care gaps.

Requirements include a BSN, an active OH RN license, and at least 2 years of RN experience in clinical settings. Telephonic triage experience is preferred.

Qualifications

• Active and unrestricted RN license in Ohio required.
• Minimum 2 years RN experience in hospital or medical practice setting.
• Telephonic nursing triage experience preferred.

Responsibilities

• Conduct telephone triage for patient health complaints using an electronic triage assessment tool.
• Navigate multiple electronic platforms to assess health complaint, chart, and scheduling.
• Document nursing triage notes in the EHR for clear documentation.
• Record phone call details in the tracking system for data collection.
• Identify and close patient care gaps.
• Collaborate with COPC staff and physicians to deliver high-quality patient care.

Description

The Triage Specialist conducts a thorough telephonic nursing triage for acute and chronic medical complaints and uses electronic health record (EHR) EPIC for documentation of patient interactions regarding triage protocols, appointment scheduling, homecare nursing instruction, and collaboration with other healthcare team members.

• Full Time/Benefits Eligible
• Monday-Friday - 8:00am - 6:00pm, 4-10 hour shifts variable days during the week and every other weekend

Essential Functions and Responsibilities:

• Conduct telephone triage for patient health complaints using an electronic triage assessment tool.
• Navigate multiple electronic platforms to assess current health complaint, patients' electronic chart, PCP appointment scheduling, and ancillary center scheduling.
• Document nursing triage notes into the EHR system for clear and concise documentation.
• Document phone call details in tracking system for department data tracking and collection.
• Assess, identify, and close patient care gaps.
• Develop positive and effective working relationship with COPC staff, offices, and physicians in order to deliver high quality patient care.

Qualifications:

Education, Licensures & Certifications

• Required: Bachelor's degree in Nursing
• Required: Active and unrestricted registered nurse license in the state of Ohio
• Required: 2 years minimum of Register Nurse experience in a hospital and/ or medical practice setting
• Preferred: Experience in telephonic nursing triage
