Job summary

SCRMC is seeking a Population Health - Telephone Triage Registered Nurse (RN) to provide patient-facing triage, education, and care navigation for hospital and clinic patients. The role operates under approved clinical protocols within Population Health workflows and aims to improve access and safety across the care continuum.

Qualifications

• Active and unencumbered RN license in Mississippi.
• Three to five years of clinical experience in acute care, emergency, or ambulatory clinic settings.
• Strong computer literacy and ability to document in EPIC and approved decision-support systems.
• Excellent active listening, verbal communication, clinical judgment, and health-literacy aware communication.

Responsibilities

• Assess patient symptoms using approved protocols and decision-support software.
• Guide patients to appropriate care levels and reinforce discharge instructions.
• Provide home-care instructions, health education, and care coordination within RN scope.
• Conduct purposeful rounding to assess understanding, safety, and care-plan needs.
• Document interactions, assessments, and recommendations accurately in the EHR.
• Coordinate follow-up and care navigation with Population Health and clinics.
