Call 4 Health is seeking an experienced Registered Nurse (RN) for a Remote Triage role supporting patients and providers nationwide. You will conduct telephone triage using Schmitt-Thompson protocols, document interactions in EHRs, and collaborate with a broad care network from a home-based setup.

The role emphasizes clinical decision-making, flexibility, and patient impact. Requires an active nursing license and multi-state licensure, with extensive RN experience in acute or ambulatory care

Formación

• RN with either an Associate's or Bachelor's degree in Nursing.
• 2–4 years of full-time RN clinical experience, preferably in ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Prior telephone triage experience using electronic triage systems.
• Experience with two or more EHR platforms.
• Active, unencumbered ENA or ENLC license and willingness to obtain additional state licenses.

Responsabilidades

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Use Schmitt-Thompson telephone triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per protocols.
• Support providers with administrative and clinical tasks (e.g., medication refills, notification of critical results, post-discharge follow-up).
• Facilitate referrals and assist with prior authorization requests needing clinical input.
• Collaborate with hospitals, nursing homes, funeral homes, and government agencies as needed.
• Participate in Remote Patient Monitoring (RPM) programs (monitoring vital signs, patient education).
• Screen and qualify patients for clinical trials and provide patient education.
