Qualifications

• Active RN license in Mississippi.

• Strong computer literacy and accurate documentation in EPIC and decision-support systems.
• Excellent active listening, verbal communication, clinical judgment, and adapting to patient health literacy.

Responsibilities

• Assess patient symptoms and urgency using approved protocols and decision-support tools.
• Guide patients to appropriate care level and convey discharge/plan instructions.
• Conduct purposeful rounding and document findings in the EHR.
• Escalate changes in condition or safety concerns to the care team.
• Coordinate follow-up and care navigation with Population Health and clinics.
