Job summary

Beth Israel Lahey Health in Brookline, MA is seeking a Team Nurse to provide direct nursing care in an outpatient primary care setting within the BIDHC network. Reporting to Practice Manager and Senior Director of Nursing, you will support MA staff and coordinate patient-centered care.

In this role you will supervise unlicensed staff, perform telephonic triage, administer vaccines and manage INR, document in the EHR, and educate patients on preventive and chronic care.

Qualifications

• Graduate from an accredited nursing program.
• RN license required or LPN license required, and CPR certification.
• 1–3 years related work experience.
• CPR Certification.

Responsibilities

• Coordinate quality clinical management across the outpatient population.
• Assist providers with exams, treatments and procedures as needed.
• Deliver nursing visits including AWVs and CCM for BIDHC patients.
• Manage INR and vaccine administration per protocol.
• Document care in the EHR and coordinate referrals.
