Job summary

Beth Israel Lahey Health's Primary Care team in Jamaica Plain, MA is seeking a Team Nurse (RN/LPN) to provide high quality patient care in a collaborative, outpatient setting. Reporting to the Practice Manager and Senior Director of Nursing, you will support MAs and clinicians to coordinate care and educate patients.

You will perform nursing duties, participate in care planning, manage vaccines and medications, and ensure accurate documentation in the Electronic Health Record.

Qualifications

• Graduate from an accredited Nursing Program required.
• Bachelor's degree preferred.
• RN license or LPN license with CPR certification required.
• 1–3 years of related work experience.
• Strong interpersonal, patient communication and education skills.
• Proficient with Microsoft Office applications.

Responsibilities

• Coordinate and facilitate quality clinical management of a broad outpatient population.
• Collaborate with providers to identify preventive care needs and care coordination.
• Provide skilled telephonic triage and direct patient care visits, including AWVs and CCM.
• Mentor and oversee unlicensed staff and Nursing Students as needed.
