Qualifications

• Graduate from an accredited Nursing Program required.
• MA RN license and BLS certification required.
• 1–3 years related work experience required; new grads considered.
• CPR Certification required.
• Strong interpersonal and patient education skills.
• Advanced skills with Microsoft applications (Outlook/Word/Excel/PowerPoint/Access).

Responsibilities

• Coordinate and facilitate quality clinical management of outpatient population across care continuum.
• Collaborate with providers to identify preventive care needs and chronic disease management.
• Provide telephonic triage and coordination of care; educate patients and families.
• Perform direct patient care through nursing visits (AWVs/CCM) for BIDHC patients.
• INR management and teaching for Coumadin patients using a standardized protocol.
• Administer vaccines/medications per orders and policies; educate patients.
• Document all patient care in EHR; schedule tests and referrals as ordered.
• Monitor abnormal test results; oversee inventory and ordering of supplies and equipment.
• Provide oversight and training to MAs/CAAs; ensure mandatory training compliance.
