Job summary

Qualifications

• Graduate from an accredited Nursing Program (required).
• Licensed Registered Nurse in Massachusetts (required).
• 1–3 years related work experience preferred; new grads considered.
• CPR certification required.
• Strong interpersonal and patient-education skills.
• Proficiency with Microsoft applications (Outlook, Word, Excel, PowerPoint).

Responsibilities

• Coordinate and facilitate quality clinical management in outpatient care.
• Collaborate with providers and community-based care partners.
• Provide skilled telephonic triage and patient education.
• Manage INR testing and Coumadin education.
• Administer vaccines and medications per orders and policy.
• Document patient care in EHR and coordinate tests/referrals.
• Monitor abnormal results and ensure timely follow-up.
• Oversee inventory and safe handling of supplies and specimens.
• Supervise unlicensed staff and provide ongoing education.
• Protect PHI and promote patient safety in daily operations.
