Beth Israel Lahey Health seeks a Team Nurse RN to support an outpatient primary care setting. You will report to the Practice Manager and Senior Director of Nursing, delivering high-quality patient care in a team-based model and coordinating with providers and MA staff.

The role emphasizes preventive and chronic care, education, and safe delegation of tasks, including INR management and vaccination administration, with thorough EHR documentation and care coordination across the continuum.

Qualifications

• Graduate from an accredited Nursing Program.
• Registered Nurse license in Massachusetts and CPR certification required.
• 1–3 years of related work experience (new graduates considered).
• Strong interpersonal, patient communication and education skills.
• Proficient with Microsoft Office and other web-based applications.

Responsibilities

• Coordinate and facilitate outpatient clinical management across the continuum of care.
• Collaborate with providers and community partners for care coordination.
• Provide telephonic triage, education and CCM visits to BIDHC patients.
• INR management and teaching for patients on Coumadin.
• Administer vaccines and medications per orders; educate patients.
• Document care in EHR and schedule tests or referrals.
• Monitor abnormal results and ensure timely follow-up.
• Oversee inventory, supplies and specimen handling.
• Educate and supervise MA staff and nursing students.
