Job summary

BETH ISRAEL LAHEY HEALTH is seeking a Team Nurse to provide high-quality nursing care in an outpatient primary care setting. You will support the MA staff and act as a clinical resource under the Practice Manager and Senior Director of Nursing.

You will coordinate care across the continuum, perform telephonic triage, wound care, INR management, and vaccination administration, while focusing on patient safety and effective communication with providers and patients.

Qualifications

• Graduate from an accredited Nursing Program required.
• License as Registered Nurse or Licensed Practical Nurse required, and Basic Life Support certification.
• 1-3 years related work experience required.
• CPR Certification required.
• Strong interpersonal and patient education skills.
• Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) or similar tools.

Responsibilities

• Coordinate and facilitate quality clinical management of a broad outpatient population across the continuum of care.
• Collaborate with providers to identify needs for preventive care, chronic disease management and care coordination.
• Provide educational activities for patients and staff.
• Perform telephonic triage and assess care needs; educate and coordinate resources.
• Manage INR and patient education for Coumadin therapy.
• Perform nursing activities such as wound care and point-of-care testing; document in EHR.
• Schedule tests and referrals, and monitor abnormal results.
• Oversee inventory and procurement of supplies; ensure PHI is used appropriately.
