You will work under the Practice Manager and Senior Director of Nursing, supporting MAs and staff, providing education, and coordinating care across the continuum.

Qualifications

• Graduate from an accredited Nursing Program.
• MA RN license required.
• 1–3 years related work experience preferred.
• CPR certification and BLS required.
• Strong interpersonal and patient communication skills.
• Proficient with Microsoft Office and basic clinic software.

Responsibilities

• Coordinate and facilitate quality clinical management in outpatient care.
• Collaborate with providers to identify preventive and chronic care needs.
• Provide triage via phone and coordinate care with community resources.
• Perform direct patient nursing care and assist with exams and procedures.
• INR management and patient education for Warfarin/Coumadin patients.
• Administer vaccines and medications per provider orders and BIDHC policy.
• Document care in EHR; schedule tests and referrals as ordered.
• Monitor abnormal test results and oversee inventory and supplies.
