Job summary

Qualifications

• Graduate from an accredited Nursing Program required.
• Bachelor's degree preferred.

Responsibilities

• Coordinate and facilitate quality clinical management of a broad outpatient population.
• Collaborate with providers on preventive care and chronic disease management.
• Provide skilled telephonic triage and coordination of healthcare resources.
• INR management and patient education for those on Coumadin.
• Administer vaccines and medications per orders and policy.
