Job summary

Yellowstone Boys and Girls Ranch seeks a Targeted Case Manager to coordinate wrap-around supports for youth and families across Montana. You will advocate, assess needs, refer to services, and help implement service plans in collaboration with a treatment team.

Starting hourly pay ranges from $20.38 to $22.94, based on education and experience, with a 11-step pay schedule. Requires a bachelor’s degree in human services and background checks.

Qualifications

• Bachelor’s degree in human services or related field.
• Criminal/Child Abuse Clearance required.
• 21+ years old.

Responsibilities

• Advocate, support, and guide youth and families in obtaining needed services, including creating and implementing service plans.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to identify best options for youth.
• Collaborate with youth, family, and professionals to develop treatment plans.
• Complete and maintain documentation per COA, Montana ARM's, contracts, and YBGR policies.
• Prepare paperwork and ensure PQI and program financial viability.
