Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to coordinate services for youth and families. You will advocate, assess needs, and collaborate with schools, families, and providers to implement care plans.

Documentation and PQI compliance are essential as you navigate a multi-agency system in Montana. Starting hourly pay ranges from $20.38 to $22.94, with experience and education determining placement on the 11-step pay schedule.

Formación

• 21 years old or older.
• Criminal / Child Abuse Clearance: must pass background checks as required by Montana.
• Bachelor’s degree in human services or equivalent experience considered.
• Ability to relate therapeutically with youth and families and maintain confidentiality.
• Ability to work with youth in school, home, and community settings.
• Excellent attendance and reliability.
• Typing 35+ wpm; familiarity with Microsoft Office 365.

Responsabilidades

• Advocate, support, and guide youth and families to obtain needed services.
• Assess needs, refer to services, create and monitor service plans.
• Maintain contact with service providers to meet youth needs.
• Collaborate with youth, family and professionals to develop treatment plans.
• Complete, update and maintain documentation per COA, MT ARM, contracts, and policy.
• Submit and verify paperwork while ensuring PQI and program viability.
