Regional Center of the East Bay is seeking staff to coordinate IPP/IFSP planning for individuals with developmental disabilities in Alameda and Contra Costa counties. You will oversee service implementation, monitor health and safety, and maintain documentation in compliance with policy.

Formación

• BA degree in Social Work, Psychology or related human services field with 2 years experience OR BA in an unrelated field with 4 years direct experience with individuals with developmental disabilities.
• Ability to communicate clearly and effectively, verbally and in writing.
• Automobile and valid driver’s license required.
• Demonstrate cultural awareness and sensitivity with diverse populations.
• Ability to speak Tagalog.

Responsabilidades

• Facilitate planning process and develop IPP/IFSP for assigned Consumers; monitor and evaluate services.
• Coordinate support for implementation of Consumer plans; identify supports and funding needs.
• Advocate for consumers to protect rights and ensure entitlement to services.
• Assist consumers and families in obtaining diagnosis/evaluation and obtaining reports.
• Monitor health and safety of consumers; maintain required documentation and contacts.
• Implement RCEB policies and procedures; complete reviews in a timely manner.
• Keep supervisor informed of caseload and unmet needs in the community.
• Perform other duties as assigned.
