Bay Area Rehabilitation Center in Baytown, TX, seeks a dedicated Case Manager to coordinate services for individuals with disabilities. You will assess needs, develop care plans, connect clients with providers and waiver programs, monitor progress, and advocate for access to supports.

The role requires an Associate degree and 2+ years of case management experience, plus strong communication, organization, and HIPAA documentation. CPR/First Aid is a plus; bilingual skills are helpful.

Qualifications

• Associate degree in Social Work, Human Services, or a related field.
• 2+ years of case management or care coordination experience.
• 1 year of Health and Human Services Waiver Programs experience.
• Knowledge of social work principles, rehabilitation services, and community resources.
• Ability to assess needs, develop service strategies, and follow through on care plans.
• Strong communication and interpersonal skills.

Responsabilités

• Assess clients' needs and identify appropriate services and supports.
• Develop individualized care plans and goals.
• Coordinate services with healthcare providers, community resources, and HHS Waiver Programs.
• Monitor progress through follow-ups, home visits, and reassessments.
• Advocate for clients and help navigate service systems and transitions.
• Educate clients and families about agency services, eligibility, and procedures.
• Collaborate with interdisciplinary teams and partners.
• Maintain confidential case records per HIPAA and policies.
• Support the mission of promoting independence and equitable access.
