Qualifications
- BA/BS degree in human services or related field and two years of experience.
- Bilingual Spanish/English highly desired.
- Detail-oriented and highly organized; strong communication skills.
- Must have valid driver's license and reliable transportation.
Responsibilities
- Provide overall service coordination and advocacy for consumers and families.
- Develop and implement IPP and PCP plans and progress reports.
- Prepare annual reviews and reassess progress and program effectiveness.
- Identify community resources and refer to vendored services as needed.
- Monitor service delivery and ensure compliance with regulations.
