Regional Center of the East Bay (RCEB) seeks bilingual Case Managers I and II to support individuals with developmental disabilities. The role covers full case management, IPP/IFSP planning, and service coordination.

You will advocate for clients, ensure access to necessary evaluations, maintain documentation, and monitor health and safety while working with diverse communities. Tagalog language ability is required.

Formación

• BA in Social Work or related field with 2 years of relevant experience.
• Master's degree required for Case Manager II.
• Ability to speak Tagalog is required.
• Valid driver's license and automobile required.

Responsabilidades

• Oversee the complete case management process from assessment to documentation.
• Develop IPP/IFSP plans and monitor service delivery.
• Coordinate supports and evaluate needs for funding.
• Advocate for consumer rights and ensure entitlements.
• Assist with diagnosis/evaluation and secure reports.
• Monitor health and safety of consumers.
• Maintain required documentation and communicate with supervisor.
• Perform other duties as assigned.
