Job summary

Tri-Counties Regional Center in San Luis Obispo is seeking a Service Coordinator II to provide person-centered consultation, advocacy and service coordination for people with developmental disabilities.

The role involves performing IFSP/IPP planning, joining IDT staffings, and coordinating services with community resources. A BA/BS in human services, 2 years of case management, and bilingual English/Spanish are required, along with a CA driver’s license and travel flexibility.

Qualifications

• Requires Master’s or Bachelor’s in human services or related field and 2 years of applicable experience in case management.
• Must be able to read/speak/write Spanish and English.
• Driver’s license and reliable vehicle; willingness to travel as needed.

Responsibilities

• Conduct strength-based assessments and involve families in person-centered planning.
• Design, document, implement and monitor services and supports.
• Coordinate services and secure funded supports and resources.
• Provide information and referrals to community services and agencies.
• Participate in interdisciplinary team (IDT) staffings and community meetings.
