Regional Center of the East Bay seeks Case Managers to lead the full case management cycle, including assessment, planning, advocacy and documentation for consumers in Alameda and Contra Costa counties.

The role requires a BA in Social Work or related field with experience; or a BA in another field plus four years’ direct work with developmentally disabled individuals; Mandarin language ability is a plus. Competitive benefits included.

Formación

• BA Degree in Social Work or related field and 1 year similar experience OR BA in an unrelated field plus 4 years experience with the developmentally disabled.
• Ability to communicate clearly and effectively, verbally and in writing.
• Automobile and valid driver’s license required.

Responsabilidades

• Facilitate planning and develop IPP/IFSP for Consumers and oversee implementation.
• Coordinate and support Consumers' plans with natural supports and funded services.
• Advocate to protect consumer rights and ensure entitlements.
