Job summary

Care Lync in the United States is seeking a dedicated Support Coordinator to develop individualized care plans, coordinate services, and monitor outcomes for adults with disabilities enrolled in Medicaid waivers. You will broker housing, medical, social, and community supports to help clients live independently.

Responsibilities include completing assessments, creating person-centered plans, ensuring ISP alignment with behavioral and crisis plans, and reporting incidents.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan for each participant and modify annually or when there is a significant change.
• Ensure the participant choice of providers by providing information for an informed decision.
• Ensure the Behavioral Support Plan and Crisis Plan is consistent with the ISP.
• Report all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
• Actively coordinate with other individuals and entities essential in care delivery to ensure seamless coordination.
• Responsible for researching and developing alternative solutions to participants needs.
• Ensures confidentiality regarding sensitive material and private health information.
• Attends required trainings and recertification classes.
• Confers with supervisor to understand participants' needs and abilities.
• 50% travel to participants as needed
