Job summary

Care Lync is seeking a motivated Support Coordinator / Service Coordinator / Case Manager to develop individualized care management plans and coordinate services for adults in the Medicaid Waiver program. The role focuses on navigating and securing services to support independent living in home and community settings.

You will brokering services, monitor outcomes, and ensure professional, cost-effective service delivery while maintaining confidentiality and complying with program regulations.

Qualifications

• BS/BA degree in related course work is required or preferred.
• Experience with Microsoft Office and state-maintained databases.
• Maintain proficiency in company-sponsored training and certifications.
• Maintain state-related clearances.
• Complete required training courses as regulations require.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or when there is a significant change.
• Ensure participant choice of providers by giving information for an informed decision.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report incidents of abuse, neglect, or exploitation immediately to supervisor/director.
• Coordinate with other care coordinators to ensure seamless care delivery.
• Research and develop alternative solutions to participants' needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertification classes.
• Communicate with supervisor/ directors about participant needs, abilities, and limitations.
• Travel up to 50% to participants as needed.
