Job summary

Care Lync is seeking a dedicated Support Coordinator to coordinate case management for adults with intellectual/developmental disabilities and autism enrolled in Medicaid Waiver. You will develop individualized care plans, broker services, and monitor outcomes so clients can live independently in their homes and communities.

Responsibilities include assessments, ISP development, coordinating with providers, ensuring confidentiality, and traveling to participants as needed (up to 50%).

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office, state-maintained databases.
• Maintain state related Clearances.
• Complete and maintain required trainings/certifications.
• Valid Driver's License with vehicle insurance and registration on file.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or with significant changes.
• Ensure participant choice of providers with information for informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report all incidents of abuse, neglect, or exploitation to supervisor.
• Coordinate with other care coordinators to ensure seamless delivery of services.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertification classes.
• Travel to participants as needed (up to 50%).
