Job summary

Care Lync is seeking a dedicated Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources. You will build Individual Support Plans and advocate for participants while collaborating with medical professionals and families.

Role includes travel to participants’ homes or communities up to 50%. We offer professional development, mentorship, and clear pathways for growth within social services.

Qualifications

• Strong organizational and communication abilities are required.
• Advocacy and person-centered support experience
• Willingness to travel up to 50% to meet participants in homes and communities

Responsibilities

• Conduct person-centered assessments to identify needs, goals, and preferences.
• Develop and update Individual Support Plans (ISP) for participants.
• Provide choice and advocacy in selecting providers and care options.
• Coordinate with medical professionals, behavioral specialists, family, and community resources.
• Monitor services to ensure safety, satisfaction, and progress toward independence.
• Assist individuals with disabilities and aging participants to access community resources.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
