Job summary

Care Lync in Maine is seeking a Support Coordinator to help adults with disabilities and aging individuals access housing, medical care, and community resources. You will conduct person-centered assessments and develop Individual Support Plans, coordinating with healthcare professionals, families, and local providers.

This role offers professional development, mentorship, flexible pathways for advancement, and a strong commitment to independence and dignity.

Qualifications

Bachelor's degree is required in a related field.
Strong organizational, communication, and advocacy skills are essential.
Ability to travel up to 50% to meet participants in homes and communities.

Responsibilities

Conduct person-centered assessments to identify each participant's needs, preferences, and life goals.
Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services.
Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
Support both individuals with disabilities and aging adults in accessing the community resources they need.
Maintain documentation, confidentiality, and compliance with all program and ethical standards.
Participate in ongoing training and professional development to strengthen skills and impact.
